We're glad you're here. We serve the health care needs of all students enrolled at Western Michigan University. Every WMU student is eligible to receive services at Sindecuse Health Center whether they are insured or not. Please explore this site to discover the many services and health education opportunities available.

Incoming students should take these steps before arriving at WMU:

U.S. students

Download U.S. student forms 

Download the package with forms to:

  • Authorize treatment if a student is under 18 (parent or guardian signature required)
  • Fill out a health insurance information form (if insured)
  • Make a photocopy of your insurance card front and back

International students

Download international student forms 

Download the package with forms and TB guidelines to:

  • Authorize treatment if a student is under 18 (parent or guardian signature required)
  • If you have a private insurance plan, make a copy of your benefits summary in English

Review insurance requirements to see if your plan meets the University's required minimum coverage amounts.

All students

Complete the forms and return all required health documents to health center staff at Parent & Family Orientation events, drop them off in person at the health center or mail to:

Immunizations

We strongly encourage verification of immunization status for all students. Please review our recommended immunizations to protect personal health and the health of the Western Michigan University community.
