Welcome New Students

 

We're glad you're here. We serve the health care needs of all students enrolled at Western Michigan University. Every WMU student is eligible to receive services at Sindecuse Health Center whether they are insured or not. Please explore this site to discover the many services and health education opportunities available.

Incoming students should take these steps before arriving at WMU:

Download the package with forms to:

  • Fill out a patient insurance information form (if insured)
  • Authorize treatment if a student is under 18 (parent or guardian signature required)
  • Make a photocopy of your insurance card front and back
  • (Optional) Authorize release of information for communication purposes from the health center to a parent or family member if you choose. Follow the example shown. The student-patient must sign the form.

Sindecuse staff will attend the Faculty, Parent and Family mixers on the afternoon of the first day of Orientation from 3:30 to 4:30 p.m. to answer questions and collect completed forms. We look forward to meeting you.

International students

Download the package with forms and TB guidelines, then:

  • Learn about TB testing requirements and gather the results of your TB tests in English
  • Authorize treatment if a student is under 18 (parent or guardian signature required)
  • Fill out a patient insurance information form
  • Make a photocopy of your insurance card front and back

WMU recently adopted a policy regarding health insurance which affects all students. International students are invited to explore some health insurance options .

Complete the forms and return all required health documents to health center staff at Parent and Family Orientation events, drop them off in person at the health center or mail to:

Immunizations

We strongly encourage verification of immunization status for all students. Please review our recommended immunizations to protect personal health and the health of the Western Michigan University community.
