This section applies to certain employees working at the University that qualify for medical and prescription drug coverage under the Affordable Care Act (ACA), however, they do not have full benefits eligibility. These employees have the option to elect certain benefits during Open Enrollment for calendar year 2018. Enrollment in these benefits is contingent upon employment with the University as of Jan. 1, 2018.
The Healthy Blue Living Health Maintenance Organization (HMO) medical plan option through Blue Care Network (BCN) will continue to be offered in 2018. A new Community Blue Preferred Provider Organization (PPO) health plan will be offered and it is administered by Blue Cross Blue Shield of Michigan (BCBSM).

You may elect the Healthy Blue Living HMO plan during Open Enrollment by completing the Health Insurance Enrollment and Change form. You must also designate a Primary Care Provider (PCP) on this form. To locate an in-network provider, go to bcbsm.com, then click on Find a Doctor (search without logging in and enter Blue Care Network under Your Plan, then change Your Location).
In order to have coverage in 2018, you must complete and submit a Health Insurance Enrollment and Change Form.

What forms do I need to complete?
To change your health care benefit election effective Jan. 1, 2018, complete the health insurance enrollment and change form:
- PPO Health insurance enrollment and change form (complete electronically)
- PPO Health insurance enrollment and change form (print to complete)
- HMO Health insurance enrollment and change form (complete electronically)
- HMO Health insurance enrollment and change form (print to complete)
To enroll a designated eligible individual effective Jan. 1, 2018, complete the designated eligible individual enrollment form:

Unable to print forms?
If you are not able to print the enrollment forms, you may pick up a copy of the form(s) in Human Resources.
Where do I send my completed forms?
Submit your completed health insurance enrollment and change form to Human Resources no later than 5 p.m. on Thursday, Nov. 30, 2017.
When will my changes take place?
Health care benefit elections are effective Jan. 1, 2018. Monthly billing will start in January 2018.
