Long term disability (LTD) claims

Reporting your long term disability claim

Timely reporting of your long term disability claim is critical to continuation of income. If you have been out for 15 days or expect to be out of work for more than 30 days you must file a claim with Matrix. To initiate your claim, phone Matrix toll free at 1-866-533-3438 or report online at www.matrixeservices.com. The Matrix Claims Intake Center is available twenty-four hours a day seven days a week.

When reporting your claim, you will be asked to provide some basic information, including:

  • Personal information - name, address, telephone number, and the last four digits of your Social Security number.
  • Job information - job title, job description, workplace location and address, work schedule, supervisor's name and telephone number, date of hire, and last day worked.
  • Illness/injury information - nature of the illness, how, when an, if applicable, where the injury occurred, the date your disability began and when the disability commenced.
  • Physical information - name, address, telephone number, and fax number for each treating physician.

Having this information readily available when you call Matrix will make the intake process much faster. The intake coordinator will provide you with any other necessary instructions.
