The 2013 open enrollment period will begin on Monday, Nov. 11, and conclude on Friday, Dec. 6. All enrollment and change forms must be submitted to Human Resources by 5 p.m. on Friday, Dec. 6.

The University's Community Blue PPO health plan is administered by Blue Cross Blue Shield of Michigan. Health care benefits also include dental, vision, and prescription drug coverage. Submit your completed Health Insurance Enrollment and Change form to Human Resources no later than 5 p.m., Friday, Dec. 6, 2013.
