Official transcripts from ALL the colleges or universities you attended. Photocopies and transferred courses listed on another transcript will not suffice. Schools are to send official transcript to the Office of Admissions.
3. GRE Scores
GRE General Test
Subject test scores are not required.
Send one set of scores using these codes:

Psychology Code: no code
(Psychology Department requires GRE scores for both MA & Ph.D.)

Additional criteria (described in above linked brochure) include previous research activity, professional experience, letters of recommendation, and a personal statement of the applicant's professional goals and prior training.
The Industrial Organizational Program requires a supplemental application:
MS Word or .pdf format. 
The Clinical Program requires a writing sample. More details on the Clinical application requirments can be found here.
