Completion of the Physical Activity Readiness Questionnaire (PAR-Q) is required prior to initial participation. PAR-Qs may be filled out at the SRC Service Desk at time of payment.
All fitness passes are to be used only by the person who purchased the pass. You may not transfer it or allow anyone else to use your pass or punches.
