What is the care management process?

A care manager will first visit the patient's residence and complete a comprehensive needs assessment with the patient/family. During this assessment, our care manager will determine the patients current status to aid in development of a care plan. The care plan is created and monitored by our care managers and Registered Nurse (R.N.), and outlines the specific care to be provided to the individual. It is a documented plan of action that our care management team will implement to support or resolve diagnosis' or ailments, examine risk factors, and create goals to maintain health. In addition to weekly or biweekly visits from the care manager, the R.N. will visit on a monthly basis. Our care managers work with the families of older adults and communicate changes in care with the patients care team.
