Attending COVID-19 Vaccination appointments

Sindecuse Health Center has secured approval to be a COVID-19 vaccination site and the health center has a process in place to administer it based on the Michigan Department of Health and Human Services phasing strategy.

If you've made a vaccination appointment, note the location and please keep these things in mind on the day of your appointment:

Complete your questionnaire in the patient portal. Everyone scheduled for a vaccination appointment will be required to complete a questionnaire before a vaccination. It will be available in the patient portal, and gives your consent for us to administer the vaccine.

If your appointment is at the health center, check-in 15 minutes before your appointment.

Wear a mask, stay socially distant while here. We have charted a path through the center which allows for social distancing. There are necessary stops along the way so give your fellow attendees space while you're here. 

Have your daily screening survey ready. Have the 'green badge' open on your phone. We'll be checking it before you enter the clinic. Keep it open for easy identification at stations along the vaccination path.

Wear loose sleeved-clothing. For this event, we cannot offer the kind of privacy you associate with an exam room. Vaccinations will be administered in an open area. Wear loose-sleeved upper clothing so the nurse can easily access your shoulder without disrobing.

Prepare to wait 15 to 30 minutes after receiving a dose. Most patients will wait 15 minutes after the shot in case an adverse reaction occurs. Some patients with specific indications will be asked to wait 30 minutes.
