Affordable Care Act (ACA) Eligible employees are those that work at the University and qualify for medical and prescription drug coverage under the ACA, who do not have full benefits eligibility. If you are an ACA Eligible employee, you have the option to elect certain benefits during Open Enrollment for the calendar year 2019. Enrollment in these benefits is contingent upon employment with the University as of Jan. 1, 2019.
A Community Blue Preferred Provider Organization (PPO) health plan is offered and it is administered by Blue Cross Blue Shield of Michigan (BCBSM). The Healthy Blue Living Health Maintenance Organization (HMO) medical plan option through Blue Care Network (BCN) will also be offered in 2019.
Your benefit elections do not carry over to 2019. In order to have coverage, you must complete and submit a Health Insurance Enrollment and Change Form.

What forms do I need to complete?
To change your health care benefit election effective Jan. 1, 2019, complete either the PPO or HMO health insurance enrollment and change form.
To enroll a designated eligible individual effective Jan. 1, 2019, complete the designated eligible individual enrollment form.
Where do I send my completed forms?
Submit your completed health insurance enrollment and change form to Human Resources no later than 5 p.m. on Friday, Nov. 30, 2018.
If you are not able to print the enrollment forms, you may pick up a copy of the form(s) in Human Resources.
When will my changes take place?
Health care benefit elections are effective Jan. 1, 2019. Monthly billing will start in January 2019.
