Designation Request Form
Designation Request Form description:
The WMU Signature Designation Request Form is located in ExperienceWMU and is used to request a WMU Signature designation on your academic transcript and diploma only. Requested information includes: Name, Western Identification Number (WIN), WMU Email Address, Graduation Semester, and WMU Signature Pathway. Even with approval, this form does not guarantee a WMU Signature designation, which is contingent upon completion of the WMU Signature requirements.
WMU Signature Designation Requests received after the deadline may not be accepted.
There is no cost for the WMU Signature designation request.
This is separate from the graduation application required by the University. Information on how to submit your graduation application can be found on the Registrar’s Office website.
