Medical, Prescription Drug and Dental
Typically, in-network providers submit your claim for you; some out-of-network providers may also do so. For claims that you must submit yourself, send them to:

Pre-addressed claim envelopes are available for pick-up in Human Resources. Claim forms are available at Forms: Benefit Claims.
Vision
Vision Service Plan (VSP) network providers file claims directly with VSP. Members with a claim from an out-of-network provider must file it themselves with VSP. Claim forms and instructions are available at Forms: Benefit Claims.
