Appointment Phase Procedures

The department prepares the letter of offer, using appropriate form letter.

The department sends the letter of offer as an electronic document to the dean's office; it is then forwarded to the office of the provost.

The provost must approve the letter prior to any commitments being made—refer to the summary of hiring procedures, part 6.

Upon receiving the candidate’s acceptance, the department processes final hiring packet to hire the individual. The hiring packet is forwarded through the dean to the provost. The hiring packet consists of: 

  • Signed letter of offer
  • P-008
  • Signed faculty credential summary
  • Vita
  • Biosketch
  • Inventions, Proprietary Information and Other Intellectual Property Agreement
  • Faculty Salary Option Form (If AY faculty only)

Upon receipt and processing by human resources, the department will receive a Faculty Appointment Form confirming the appointment.
