Internal Therapy Dog Request

We are pleased to offer the support of therapy dogs at Unified Clinics. Dr. Angie Moe, a professor of sociology at Western Michigan University and director of our Therapy Dog Clinic, is the trainer and handler for three registered therapy dogs, Sunny, Oreo and Poppy. All have gone through extensive training and preparation in order to assist our efforts. 

This form is a tool to help us better assess your request. Completion of this form does not ensure that we will be able to partner with your clinic but will help us determine the best plan of action. Please note that requests must come from a representative of Western Michigan University. After you complete the request, Dr. Moe will contact you to discuss further.
