General summary
Provides community support coordination services to consumers with disabilities. Supervises direct care staff.
Major duties
- Schedules, trains and supervises direct care staff, including student employees and interns. Monitors community locations for service quality. Participates in hiring decisions.
- Develops and coordinates community activities for assigned consumers. Coordinates logistical aspects of community integration activities.
- Provides direct care of consumers in community locations and provides for or ensures the personal care needs of assigned consumers are being met.
- Ensures the accurate completion of documentation on assigned consumers and assures maintenance of case records.
- Resolves team decisions and consumer issues as necessary, includes assisting in the development of and managing of behavior treatment plans.
Minimum qualifications
Minimum qualifications based upon job documentation and industry best practices. Any current employees not meeting these qualifications will be grandfathered until they move to a different job.
Required education
- High school diploma or equivalent.
Required experience
- Two years’ relevant experience.
- Direct support assistance experience.
Physical requirements
Office or other indoor work with minimal physical demands such as occasionally lifting or moving materials less than 25 pounds.
Working conditions
Work is generally performed in a well-lit, temperature-controlled indoor environment with occasional exposure to the outdoors or any number of elements.
