General summary
Serves as a case manager.
Major duties
- Serves as a case manager. Completes in-depth reports, including assessments, diagnosis, recommendations, activities and interventions.
- Conducts pre-assessment data collection on adults with developmental disabilities and other mental health conditions. Reviews various clinical and other relevant records and makes behavioral observations.
- Participates in the interdisciplinary assessment process, including interviews, follow-up meetings and reassessments.
- Assists with the development, review and monitoring of an individualized budget. Assists in accessing and maintaining sources of financial support and eligibility, including but not limited to Supplemental Security Income, Social Security Disability Insurance and food stamps.
- Provides active and on-going follow-up with clients and families to facilitate recommended interventions. May participate in medical consultations. Serves as an advocate and provides information to other community organizations.
- Develops individual back-up plans for essential support services for emergencies or unforeseen circumstances.
- Develops, organizes and conducts group informational sessions for families that target family and client concerns.
- Supervises staff.
Minimum qualifications
Minimum qualifications based upon job documentation and industry best practices. Any current employees not meeting these qualifications will be grandfathered until they move to a different job.
Required education
- Bachelor’s degree in related field from an accredited institution.
Required experience
- Five years' relevant experience.
- Supervisory or lead experience.
- Strong interpersonal, written and verbal communication skills.
- Experience working with adults with developmental disabilities and their families, including experience in connecting individuals to their community, implementing self-determination models of support and facilitating the development of person-centered plans and individualized budgets.
Physical requirements
Office or other indoor work with minimal physical demands such as occasionally lifting or moving materials less than 25 pounds.
Working conditions
Work is generally performed in a well-lit, temperature-controlled indoor environment with occasional exposure to the outdoors or any number of elements.
