General summary

Provides case manager support.

Major duties

  • Assists in completing in-depth reports, such as assessments and person-centered plans, including recommendations, activities and interventions.
  • Assists with scheduling and facilitating the interdisciplinary assessment and planning process for a select group of clients, including follow-up team meetings and periodic reviews.
  • Assists the Case Manager in visiting clients and taking clients to appointments. Monitors services as needed and links to community resources.
  • Facilitates connection to community and meaningful activities.
  • Provides clerical support to the Case Managers including filing, copying and mailing of documents.

Minimum qualifications

Minimum qualifications based upon job documentation and industry best practices. Any current employees not meeting these qualifications will be grandfathered until they move to a different job.

Required education

  • Bachelor's degree in related field from an accredited institution.

Required experience

  • One year's relevant experience.
  • Experience with or an understanding of persons with developmental disabilities.

Physical requirements

Office or other indoor work with minimal physical demands such as occasionally lifting or moving materials less than 25 pounds.

Working conditions

Work is generally performed in a well-lit, temperature-controlled indoor environment with occasional exposure to the outdoors or any number of elements.
