General summary

Coordinates the Healthy Lifestyles initiative as a core program at the Center for Disability Services.

Major duties

  • Coordinates the Healthy Lifestyles initiative as a core program at the Center for Disability Services and provides leadership for the program.
  • Ensures all grant outcomes and deadlines identified in the Healthy Lifestyles curriculum are met.
  • Develops a network of community partner agencies to participate in the grant project.
  • Develops schedules. Coordinates consumers, staff, location and transportation for the series of classes. Coordinates student events and health professional consultants to conduct classes and assessments.
  • Develops and organizes consumer education material and classes.
  • Writes all grant reports. Coordinates all project evaluation activities and present evaluation results to funding sources on an annual basis. Writes continuation grant proposals.

Minimum qualifications

Minimum qualifications based upon job documentation and industry best practices. Any current employees not meeting these qualifications will be grandfathered until they move to a different job.

Required education

  • Master's degree in related field from an accredited institution.

Required experience

  • Three years’ relevant experience.
  • Programmatic experience to influence community health outcomes.

Physical requirements

Office or other indoor work with minimal physical demands such as occasionally lifting or moving materials less than 25 pounds.

Working conditions

Work is generally performed in a well-lit, temperature-controlled indoor environment with occasional exposure to the outdoors or any number of elements.
