WMU Health Services Plan
A number of services are available to Western Michigan University Community Blue PPO Participants at Sindecuse Health Center and Unified Clinics.
Sindecuse Health Center
- Allergy injections, covered at 100 percent.
- Immunizations, many covered at 100 percent.
- Laboratory and radiology tests, covered at 100 percent (excludes genetic testing).
- Massage therapy, for eligible employee groups, covered through the Community Blue PPO. See Your Benefit Chart at www.bcbsm.com for more information.
- Nutrition counseling, covered at 100 percent.
- Office visits, covered at 100 percent.
- Camp or sports physical.
- Health maintenance exam.
- Gynecological exam with pap smear.
- Mammography screening referral.
- Prostrate specific antigen test.
- Office consultation.
- Clinic appointments for illness or injury.
- Orthopedic surgeon, sports medicine, podiatry, pediatric sports medicine visits, covered at 100 percent.
- Physical therapy visits and supplies, covered at 100 percent.
- Prescription drugs, covered under WMU's prescription drug plan with a three-tieir co-pay through Blue Cross Blue Shield's 90-day retail program. See your benefit chart at www.bcbsm.com for more information.
Unified Clinics
Services in the following clinical areas are available at no cost to WMU PPO plan members at the level of service defined by the clinical provider. There are no co-payments or deductibles on most services unless indicated.
- Aphasia services.
- Caregiver support groups.
- Diagnosis and treatment of problems resulting from aphasia.
- Group therapy.
- Audiology services.
- Hearing aid evaluation.
- Hearing aid selection, fitting and management (hearing aid not covered by WMU PPO health plan).
- Hearing screening and diagnostic hearing evaluation.
- Behavioral health services.
- Assessment for substance abuse, mental health or other behavioral health challenges.
- Outpatient counseling (includes individual and group counseling).
- Specific services for young adults, women and families experiencing substance abuse or mental health problems.
- Child trauma assessment.
- Interdisciplinary assessment for children exposed to potentially traumatic events (includes assessment of functioning within the cognitive, affective, behavioral and physiological domains).
- Comprehensive women's health (board certified gynecologist).
- Annual gynecological exams.
- Colposcopy.
- Injections.
- Mammography screening referrals.
- Office visits.
- Speech and language therapy.
- Evaluation and diagnosis of communication problems.
- Individual and group speech and language therapy.
- Preschool language intervention services.
- Voice and respiration services.
- Comprehensive voice evaluations.
- Diagnosis and treatment of disorders of voice.
- Vocal screenings and evaluations.
- Occupational therapy.
- Group and individual therapy.
- Functional capacity evaluation.
- Occupational therapy evaluation.
- Vision.
- Eye exams.
- Eyeglass frame sales (up to $150 included).
- Evaluation for low vision devices to enable independence in daily tasks (equipment not included).
- Contact lens exams and prescriptions.
