In-processing

Once you have registered for your first military science and leadership class, please complete the following forms and turn them in to your instructor on the first day of class. Below are instructions to help you correctly complete each form.

Fill out as much of the form as possible.

Part I: General Information

  • 3. College ID #: If you attend WMU, this is your WIN #. If you attend a different college or university, it is the identification number given to you by your school.
  • 4. Email: Enter your school email or an email address that you check regularly.
  • 5. Local address: This is the address that you reside at while attending school.
  • 7. Permanent address: This is the address that you reside at when you go home to for holidays and summer break.
  • 10. POB: This is your place of birth. Please enter the city, county and state.
  • 12. Blood type: If you do not know your blood type, please skip this box.
  • 24. Next of kin: In addition to a full name, please include your relationship to your next of kin (mother, father, grandparent, etc.).

Part II: Academic Information

  • 26. School of attendance: If you attend a school other than WMU, please fill in this box with the name of that school.
  • 27. Residency status: Indicate whether you are considered an “in-state” or “out-of-state” resident for the school that you attend.
  • 34. CGPA (college): If this applies to you, please fill in your college GPA.
  • 36. High school attended: Please also include your high school GPA.

Please skip: 26a, 31, 32, 33, 35, 35a, 37, 37a, 38, 39 and all of Part III.

Fill out as much of the form as possible.

  • 2. Pay grade: Skip this box.
  • 4. I acknowledge and understand that as a recruit or trainee, I will not: Initial next to the letter for each statement a through h.
  • 9. Violations: Initial.
  • 10. Approved by: Skip this box.

You do not need your dentist to sign this form. However, please verify with their office that your dental records contain descriptive profiles, bite wing x-rays, orthodontic profiles or dental x-rays for identification purposes. If your dental records do not contain this information, you will need to schedule an appointment with your dentist.

Contracting

If you have decided that becoming an officer and leader in the Army is right for you, all of the documents you may need for contracting are below. The semester before you are eligible to contract, please turn in your paperwork to the scholarship and enrollment officer.

Remember these contracting rules:

  • You have to contract before receiving a scholarship.
  • You can contract as a Simultaneous Membership Program cadet during your sophomore year.
  • If you do not have a scholarship and are not in the National Guard or Army Reserve, you cannot contract until the start of your junior year.

    • Section 3 must be signed by your bank. If your bank is USAA, contact the scholarship and enrollment officer first.
  • W-4: Employee's withholding certificate

    • To start, contact the scholarship and enrollment officer.
  • Social Security Card (copy)
  • DD Form 214: Discharge paperwork

  • Letter of acceptance from the unit where you will be a cadet.
