Internship Documentation

Western Michigan University supply chain management majors are required to obtain professional work experience, such as internships or co-ops, prior to graduation. Please use this form to submit your documentation. Acceptable documentation includes the following:

• A signed letter from the internship supervisor at the conclusion of the internship.
• Experience WMU (SPuRS) transcript showing the internship is complete.
• A signed letter from a supply chain management faculty member approving some other type of work experience.
