Immigration Forms

F-1 and J-1 student requests must be submitted through your I3S Terra Dotta Student Portal.  This includes CPT, OPT, RCL, Extensions, Travel Signatures, J-1 work authorizations, J-1 Academic Training and all other student requests. Paper or PDF forms will no longer be accepted.  

Employment-based forms: We are in the process for moving our forms for employment-based visas such as J-1 scholars, H-1 or Permanent Residency over to Terra Dotta. In the meantime, please review the process on our Employment-Based Visa webpage and email @email access the needed forms.
