Submit Out-of-State Verification Forms

If you're applying outside of Michigan for teacher, administrator, or school counseling certification and need your program, testing, and/or certification confirmed, please complete this submission form and upload the form you need completed. Please follow the instructions below to ensure the most efficient processing time.

• Please complete your sections of the form before uploading
• Do not add social security number (SSN) to forms (if SSN is requested on the form, only add the last four digits)
• Please allow 5-7 business days for completion of this form
• Once the form is complete, we will send a copy of the completed form to you at the email address listed in the webform below (please check your junk/spam folders as WMU email often filters into those folders)
