Counselor Submission Form

This form is for counselors to submit test scores or grades for students attempting to qualify for ATYP for the 2025-26 school year. The student should be a current 6th or 7th grader (2024-25 school year). You may upload pictures or document versions of the student's test scores or grades or type in their information. Student's Latest Test Score

This should be a nationally normed test score and be the most recent score available. It does not need to be a test score from this year but that data would be the most helpful. If there are any extenuating circumstances that we should be aware of, please leave that information in the comment field below.

Student's Latest Grades

This should be a listing of all of the students grades or just their English and math grades. The most recent grades are the most helpful. If there are no grades available for this year, then last year's grades are acceptable. Please note any issues in the comments section.
