Data Access Forms
All data viewed, modified, or retrieved via a University data system is for the use of University faculty and staff in the usual conduct of the work of the University ONLY. Any other use or disclosure of this information is strictly prohibited.
All users given security access to one or more of the student systems must abide by the University policies set forth to govern use of the student systems.
Note: Access forms for Banner access vary based on security areas. Therefore, if your department/office does NOT fall under the Registrar’s Office, International Student Services or Accounts Receivable, please contact the appropriate officer.
To request access, click on the link for the appropriate form(s) and type in the information required. When done, print the form(s), sign each form and obtain the appropriate signatures. Then forward the form(s) to your security officer for processing.
