Waiver of Liability
I affirm that I am fully aware of my physical condition, and I fully realize the dangers of participating and voluntarily assume all risks associated with my participation in the Poker Walk. With this knowledge, I release forever Western Michigan University, University Recreation,  and any of its agents or employees or other participants responsible for any damage incurred while participating in any event sponsored by University Recreation. Participating in Poker Walk constitutes an agreement to allow photos of my likeness to be used by University Recreation in future publicity-related matters. All photos are property of Western Michigan University. I have carefully read this form and fully understand its contents. I am aware that this is a release of liability, and an agreement between myself and Western Michigan University, University Recreation Department and for the benefits of others described herein. I sign it of my own free will.
