Summer Camp Agreements

Agreements & Deposit Confirmation

This form is not able to be submitted without first completing the $500 deposit payment using our payment portal. Please make note of your deposit confirmation number (located on your payment receipt) and enter it where indicated. Read through the agreements thoroughly and complete the webform below. This form is to be completed by the named legal guardian for the named student camper. 

Participation Agreement: 

• I understand that at least 1 parent or guardian must be present for the duration of the non-residence session (Session 3).  For Session 1 and 2, parent participation is optional, but will require an additional fee. 
• I have made my election to receive the camp information packet either by mail or email, and I will complete this form and mail it back within 30 days. I understand this information cannot be submitted virtually. 
• I understand for Session 1 and 2, we are responsible for transporting our participant to the camp prior to its beginning, and or picking them up upon its conclusion.  If I choose to participate in the parental option, I understand transportation for the parent may not be included.  Transportation will be provided for campers for all camping related activities.
• I understand I am responsible for our transportation and lodging for the duration of the Session 3 camp. I understand that if I choose to book with the block of rooms for campers I must submit my reservation with the hotel no later than 30 days prior to the session's start date. 
• For Session 3, I understand breakfast will not be provided during the session and that I may be responsible for some meals that occur outside of camp programming and/or camp hours. I will know in advance which meals these are, but understand that due to weather and factors outside of the University's control, this may change. 

Payment Agreement: 

• I understand that by completing this form I am confirming that I've paid the $500 deposit to guarantee my spot for Aviation Summer Camp 2025. The deposit number I've entered is correctly reflecting my submitted payment. 
• I understand that my deposit goes towards the full cost of camp and that my remaining balance will be due no later than June 1st, 2025.
• I understand that I will receive final payment information once the institution has reviewed my completed registration paperwork.
• I understand that scholarships are available, but funds are limited. I have been informed that applications are reviewed on a first-come, first-served basis. 
• I understand that for Session 3 lodging, transportation and meals outside of the camp program are my responsibility and are not included in the full cost for camp. 
• If I need a payment plan, I know to contact Tom Thinnes, to arrange it. 
• I understand that if I do not fully pay the camp balance, my registration may be considered void and canceled by Western Michigan University.

Webform Agreement: 

• By submitting this form, I certify that I am the named legal guardian of the named student camper.
• I have read and understand both agreements as written.  
• If I disagree with any of the aforementioned agreements, I will indicate this on the form and the camp coordinator will contact me to discuss the matter further. 

Acknowledgement:
